Required qualifications

You must meet the required qualifications to be eligible for a provider contract. Please read the qualifications carefully before you apply.

Current and unrestricted clinical license to practice one of the following disciplines in Washington, Oregon, or Idaho:

  • Psychology
  • Clinical social work
  • Marriage and family therapy
  • Professional or mental health counseling
  • History of ethical, competent practice, as documented by your licensing authority and self-attestation.
  • Minimum of two years of full-time or four years of part-time clinical experience while licensed.
  • Proof of licensing supervision (if you have an associate license).
  • Current professional liability insurance and commercial general liability insurance in the minimum amount of $1 million per occurrence and $3 million annual aggregate.
  • An office that is wheelchair accessible and ADA compliant (in-person offices only).

  • Are able to effectively serve diverse populations with respect.
